Initial Examination

The initial examination is carried out in a separate room from the ongoing treatments (main treatment bays are pictured in the Sun Prairie and Middleton pages).  It is a very brief inspection of the teeth to determine orthodontic status of the patient.  The findings are discussed with the parents and patient and a course of action prescribed where indicated.  The most common next step is diagnostic records, which constitutes a thorough review of the case.  Often, it is necessary to set up recall visits for the patient if the most appropriate treatment would be at a later date and level of maturity of the dentition.  Both the initial and recall examinations are free of charge (unless an x-ray is necessary and would only be done with your approval).

If you have already made an appointment for an examination, we sent you an appropriate registration form (Child/Adolescent Registration up to 18 years of age, Adult/Dependent Adult Registration after 18).  If it has been misplaced, follow the links below to go to the appropriate form and chose the [Print Icon] and when finished printing use the [BACK] button to return here.
